Work Authorization- Direction to Pay

I hereby authorize the repair work herein set forth to be done by you, together with furnishing by you of the necessary parts and other material for such  repair, and agree that you are not responsible for any delays caused by unavailability or delayed availability of parts or materials for any reason ; that you neither assume nor authorize any other person to assume for you any liability in connection with such repair ; that you shall not be responsible for loss of or any damage to the above vehicle, or articles left therein, in case of fire, theft or other cause beyond our control ; that an express mechanic’s lien is hereby acknowledged on the above vehicle to secure the amount of repairs thereto ; that your employees may operate the above vehicle during the repair process.

I do hereby appoint the aforementioned business to accept on my behalf any and all checks, drafts, or bills of exchange, and to endorse all such checks, drafts, or bills or exchange for deposit to the aforementioned business account for credit on my account for repairs on my vehicle, which has been released and accepted. 

I have read and agree with the above. I hereby authorize American Dent Specialists, LLC to perform the above detailed repairs to this vehicle.
